Conditions of Unilife Member/Business Person Application
This application will be completed when Unilife International Co., Ltd. receives a Unilife member/business person application form signed (original) and a copy of ID card or government issued ID car (with identification number and photo) of the applicant and co-applicant (in case of co-application) with signature, and receives the application fee.
Applicants must be 18 years of age or older on the date of application. For those under 20 years of age, the rightful guardian must sign a consent for application.
Application fee for UB (Unilife Business Member) is 200 Baht. Application fee for UM (Unilife User Member) is 50 Baht.
If you pay the application fee and submit the application and documents by fax or electronic but do not send the true copies of the documents (as stated in e 1) to the Company within 30 days of the date you send the document by fax or electronic, the Company reserves the right to void all such information and not to return the application fee.
In order to receive all kinds of commissions from Unilife directly, you will need to attach a copy of the first page of your bookbank signed by both business persons (in case of co-application) and it must be only savings account. If the net commission is less than 100 baht, the Company will not transfer money, but it will be combined with the discounts of the following months. We will transfer commissions to you when the total accumulated amounts is from 100 baht or more. You will be charged transfer fee at the rate of the bank fee specified

You will not be forced for any purchase when reapplying until termination of status.
We will refund your money to you if you are not satisfied with products.
